Noncompliance Area: During the inspection on 6/4/26 the file for staff person #1 included a health assessment that did not state if the staff person had a physical examination and it did not include an initial tuberculin skin test and the results.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: During the inspection on 6/4/26 an infestation of ants was observed near the front door entrance and near the children's belongings. Ants were observed crawling in and near children's shoes.

Noncompliance Area: During the inspection on 5/7/26 the file records for child #1 and 3 did not include an initial health assessment. The file record for child #2 did not include an updated health assessment.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility. The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler.

Noncompliance Area: During the inspection on 5/7/26 it was determined that the facility does not have a director present at the facility site a minimum of 30 hours per week. Staff person #1 was not present for unannounced inspections that occurred on 2/26/26, 3/13/26, 4/3/26 and a renewal inspection that occurred on 5/7/26.

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week.

Noncompliance Area: During the inspection on 5/7/26 the outdoor play space was observed being unrestricted from unsafe areas or conditions. Exposed brick was observed broken and crumbling.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During the inspection on 5/7/2026 toxics (potting soil) was observed accessible to children on the window seals of the Infant/Toddler classroom and the Preschool Room.

The facility's acceptable plan of correction indicated that TSORM hung plan back up with more secured tape by 2/26/26. Current Observation: During the inspection on 3/13/26 the facility did not have a written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency posted conspicuously in every child care space.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: Continued Noncompliance Citation-During the inspection on 2/26/26 staff person #3 was observed with 5 children, the youngest child was an infant. The facility's acceptable plan of correction indicated that TSORM will assure that all children stay in their proper classroom. When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level) by 2/26/26. Current Observation: During the inspection on 3/13/26 staff person #2 was observed with 14 school agers alone, the youngest child was a young school age child.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). All staff must obtain Better Kid Care training, "Supervision: Staff: Child Ratios by 3/27/26"

Noncompliance Area: During the inspection on 3/13/26 staff person #2 was observed with 14 school age children in the school age classroom, the school age classroom capacity is 13.
