Noncompliance Area: Children in preschool class cots were not 2 feet apart.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: Facility's refrigerator in the lower level by the kitchen was not cleaned. Facility's refrigerator in 2 year old room temperature was 47 degrees.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: Two children from the preschool class were left alone in the bathroom at separate times. A volunteer was left supervising the preschool class during nap time.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.

Noncompliance Area: Child # 1 and 3 did not have date of admission on Agreement. Child #1 and 3 did not contain admission dates

Correction Required: An agreement shall specify the date of the child's admission. A child's record shall contain the dates of application, admission and withdrawal of the child.

Noncompliance Area: Child 1, 2, 3, and 4 did not include work addresses and telephone numbers of the enrolling parent.

Staff person #1 did not have National Sex Offender Registry clearance on file at the facility.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: Staff # 5 and 6 did not have two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person on file.

Correction Required: Competence in first-aid is demonstrated by the completion of training by a professional in the field of first-aid. First-aid training shall be renewed on or before expiration of certification or every 3 years, as applicable.Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

Noncompliance Area: The children's lower level bathroom had several roaches on the walls by the sinks

Correction Required: Evidence of infestation of insects or rodents in the facility is not permitted.

Correction Required: Trash that has been contaminated by human secretions or excrement shall be contained in closed, plastic-lined receptacles.

Noncompliance Area: Hot water temperature in the children's downstairs bathroom was 116 degrees.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: During nap time preschool class did not have enough lighting to supervise children. Four light bulbs were out throughout the building (preschool class, hallway, stairwell and afterschool area)

Noncompliance Area: During inspection Certification Supervisor observed 2 infants in seats next to one another, with one child sucking the other child's pacifier. Staff #1 & #2 did not accept responsibility of 5 older toddlers in the room. Both staff named the same 5 infants that were also in the same room. Staff person #3 sent a pre-school child into the office to retrieve papers alone & unsupervised.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3270.51--3270.55.

Noncompliance Area: Staff #1, #2, #3 and #4 did not have valid health assessments on file answering all of the required questions.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: During inspection, Certification supervisor observed a previously served bottle of milk on top of the refrigerator. Milk should have been discarded.

Correction Required: Food that has been previously served to a person or returned from a table shall be discarded.

Noncompliance Area: Certification Supervisor observed moth balls in a cup on the window sill within the reach of children in the preschool room.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.

Noncompliance Area: Facility downstairs restroom had a toilet that was not working at the time of inspection.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.

Noncompliance Area: Facility restrooms did not have hand washing signs posted at toilet , sink and diapering area stating hands should be washed before eating and after toileting.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: During renewal inspection on 2/26/19 certification representative asked provider to see copy of written notification to local traffic safety authorities regarding the location of the facility and its use of pedestrian and vehicular routes around the facility.

Correction Required: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the day care facility.

Noncompliance Area: During renewal inspection on 2/26/19 certification representative asked provider to see copy of the facility's safe routes/pick-up and drop-off points.

Correction Required: Safe pedestrian crossways, pick-up and drop-off points and bike routes shall be appropriately determined in the vicinity of the facility and communicated to the children and parents in writing. Written notification of safe routes shall be posted by the operator at a conspicuous location in the child care facility.

Noncompliance Area: During renewal inspection on 2/26/19 certification representative observed that the facility is using a 12 passenger van to transport children.

Correction Required: In accordance with the requirements of 67 Pa. Code Chapter 171 (relating to school buses and school vehicles), the facility may not transport a child in an 11-15 passenger van.

Noncompliance Area: During renewal inspection on 2/26/19 certification representative reviewed all staff files. Staff # 7, whose start date was 1/14/19, did not have a copy of the request for the child abuse clearance in file.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #7 may not work in a child care position at the facility until copy of request for child abuse clearance is obtained.

Noncompliance Area: Facility staff did not retrieve child from the school to attend the afterschool program. Child was left to walk to the childcare facility alone and unsupervised.

Noncompliance Area: During complaint investigation, Staff person #3 did not have a child abuse, state police, or FBI clearance on file or verification that clearances were requested. Staff person #3 stated he was just released from prison for conspiracy to robbery.

Correction Required: The operator may not allow an individual to enter the facility if the operator knows that the individual has been convicted or is awaiting trial on charges involving a crime of child abuse, child neglect, physical violence or moral corruptness.

Noncompliance Area: Certification Supervisor observed staff person #1 alone and supervising 11 children. Upon file review it was determined that staff did not qualify as an AGS. Staff person only qualifies as an aide. Staff person #3 was alone with 1 child and had no identifying information on file at the facility.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing.

Noncompliance Area: Certification Supervisor observed an Aide alone with 1 young toddler, 3 older toddlers, and 7 pre school children.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: During time of inspection, the rest equipment being used in the upstairs older toddler room and in the downstairs preschool room were not properly labeled to identify which child was assigned to which piece of rest equipment.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: During time of inspection, the staff person working in the infant room did not wash the child's hands after completing diapering procedure and had to be reminded by cert rep to wash child's hands.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: Staff person # 1 (hired 1/15/18) , indentified as an AGS did not have proof of 2 years of experience documented in her record.
