Noncompliance Area: During electronic review of several children's files on 9/24/20, cert rep observed the emergency contact forms of CHILD # 1, 2, 4 did not include enrolling parents' work address and phone number. CHILD # 4 emergency contact form did not include release persons' addresses.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: During renewal inspection on 4/8/19 certification representative reviewed all 3 staff files. Staff # 1 and 2 state police clearances stated for VOLUNTEER purposes.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1 and 2 may not work in a child care position at the facility.

Noncompliance Area: Child # 3 and child #4 did not have the address of release person documented on their emergency contact form. In addition, the operator did not sign the agreement form for child #4.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released. The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: Child # 1 (enrolled 12/20/17) did not have initial health assessment with immunization documented in their record.

Correction Required: The initial health report for an older toddler or preschool child shall be dated no more than 1 year prior to the first day of attendance at the facility. A child's record shall contain initial and subsequent health reports.

Noncompliance Area: Child # 4 did not have a current health assessment documented in their record. (last health assessment 12/21/16)

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Noncompliance Area: During time of inspection, Staff person # 2 who has been working at the facility for over 1 year had not received a written evaluation.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: Staff person # 1 (hired 2/12/18) and identified as an assistant group supervisor did not have proof of 2 years of experience working with children.

Correction Required: An assistant group supervisor shall have a high school diploma or a general educational development certificate and 2 years experience with children.A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: On 3/19/18, a teacher was changing an infant while another infant climbed onto a high step stool without help and/or supervision.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: On 3/19/18 during lunch there were 7 preschool children and 5 young toddlers present with one teacher while the other teacher left the room to make lunches and two staff were present with 2 infants, 3 young toddlers, and 5 preschool children.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30.

Noncompliance Area: During time of inspection the facility was out of compliance with staff/child ratios. There was one staff person with 1 infant, 1 young toddler and 6 older toddlers. Upon arrive there was 6 preschool children working independently reading and looking at books with no staff person present with the group.

Noncompliance Area: Upon arrival at the facility, there was one staff person crossing the street coming towards the daycare center and opened the door into the facility. Upon entry, there were 6 preschool children sitting in the middle area of facility silently reading/looking at books with no other staff present in the space. In the back are there was 1 infant, 1 young toddler and 6 older toddler with one staff person (owner/operator). The staff person that open the door stated that she had just quickly ran across the street to pick up something. Leaving the facility briefly being monitored by 1 person with 14 children.

Correction Required: At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person. The requirement for supervision on and off the facility premises includes compliance with the staff:child ratio requirements in 3270.51--3270.55.

Noncompliance Area: Child # 1 did not have names of release person documented on the agreement forms. Child # 1 also did not have health insurance policy number documented on their emergency contact form.

Correction Required: An agreement shall specify the persons designated by a parent to whom the child may be released. Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: Child #2 did not have address of release person documented on their emergency contact form.

Correction Required: Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Noncompliance Area: Staff person # 3 did not have an updated health assessment documented in their record (last health assessment dated 2/4/15. Staff person#1 (hired 4/6/17) did not have negative TB screening dates documented in her record.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Noncompliance Area: During time of inspection, names were not recorded on the bottles of the infants in care in facility.

Correction Required: Disposable nursers and bottles shall be labeled with the child`s name.
