Findings: Three children need an updated immunization record in their file. Two children need parts of the health inventory completed. Please have their parents obtain from the physician and add to the children's files. DISCUSSION ONLY: There are three children who recently turned 2 years old and one child who recently turned 12 months old. These children are required to have the lead blood test completed. Their parents should obtain the documentation from the physician if the lead blood test was already completed. If the lead blood test has not been done yet, the physician will need to conduct the test and provide the parent with the documentation to give to the provider. Lead blood tests are required at 12 and 24 months for all children who were born after 1/1/2015.
