|Findings: There was no written documentation of daily attendance during this visit. The provider stated that the attendance sheet got ruined since a child's beverage spilled on it. The provider did show the licensing specialist an attendance template that she will fill out and have parents sign at pick up today. When this is completed the licensing specialist will be notified.|
|Findings: All three children enrolled need their emergency cards updated. Two of these children are siblings. The provider will have the parents update these cards at pick up time. When these items are completed the licensing specialist will be notified.|
|Findings: The provider revised her parent handbook and needs to have her discipline policy and pet policy written out in the revised version of the parent handbook. Once she has added these to her parent handbook/ agreement, the licensing specialist will be notified,|
|Findings: Four out of five children enrolled need the lead addendum page of the Health Inventory form completed by the parent and pediatrician if appropriate. One child needs Part 1 of the Health Inventory Form completed. When these items are completed, the licensing specialist will be notified.|
|Findings: The provider informed the licensing specialist during this visit that her daughter is no longer a resident. She left in July 2017. A reminder that notifications affecting residents must be made immediately to the licensing specialist.|
|Findings: The provider will create a passive screen time policy and have the parents sign off that they have received it and are aware of it. This will be added to the parent handbook. Once the provider has this completed, the licensing specialist will be notified.|
|Findings: Tape was missing from the First Aid Kit. The provider will add this to the kit and notify the licensing specialist when this is completed.|
|Findings: The provider needs to replace the lock on the cabinet under the sink. Dish detergent is stored there and the lock on this cabinet is broken. When the provider replaces this lock, she will notify the licensing specialist.|
|Findings: Each child's emergency information form was not updated as required.|
|Findings: All health-related documentation as required was not present for each child in care.|
