Findings: During the review of children's emergency cards, there were three emergency cards that needed to be updated. One of the children's emergency cards did not contain the home address for the enrolled child. Please ensure the discussed emergency cards are completed, updated and signed by the parent. Submit a letter of correction indicating these items have been corrected.

Findings: The Licensing Specialists inspected the outdoor play area. There was a small burning barrel located in the backyard that the children could access. This burning barrel is a distance away from the actual play equipment. Located in this barrel were ashes and burnt pieces of wood. There were jagged areas on various areas of the barrel and there was a jagged hole located at the bottom of the barrel. Theses areas of the barrel could result in injury if accessed by the children. Please ensure the burning barrel is made inaccessible to the children and send a photo for corrections.

Findings: There is a trampoline located in the back yard. There are steps to the trampoline; however, they were inaccessible. The netting, however, was not secure and allowed for access to the trampoline. Ensure that the trampoline is kept inaccessible at all times and that the netting is secured in place.
