Noncompliance Area: An anonymous caller reported to the DOH that the facility/school had closed only for one day and that there were sick children in the classroom at school, and who were not being advised to get tested.

Correction Required: An operator who observes an enrolled child with symptoms of a communicable disease or infection that can be transmitted directly or indirectly and which may threaten the health of children in care shall exclude the child from attendance until the operator receives notification from a physician or a CRNP that the child is no longer considered a threat to the health of others. The notification shall be retained in the child's file. Diseases and conditions which require exclusion are specified in 28 Pa. Code Chapter 27 (relating to communicable and noncommunicable diseases).

Noncompliance Area: At present the school has 7 positive COVID-19 cases; mostly teachers and is considered a cluster.re

Noncompliance Area: During the renewal inspection child #1 emergency contact did not include information on the child's special needs.

Correction Required: Emergency contact information must include information on the child's special needs, as specified by the child's parent, physician, physician's assistant or CRNP, which is needed in an emergency situation.

Noncompliance Area: During the renewal inspection the emergency contact did not include the health insurance coverage policy number for the child.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: During the renewal inspection in several classrooms the emergency contact information was not present in the child care space for the children that were receiving care in the space.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: During the renewal inspection child #2-4 emergency contact and fee agreement was not updated within a 6-month period.

Correction Required: The parent shall update in writing emergency contact information once in a 6-month period or as soon as there is a change in the information

Noncompliance Area: During the renewal inspection the health report for child #2 did not include a record of the immunizations.

Correction Required: A health report shall include a review of the child's immunized status according to recommendations of the ACIP.

Noncompliance Area: During the renewal inspection the emergency plan needs to be updated with the evacuation location and staff need to be trained on the new plan.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: During the renewal inspection certification rep observed a water bug crawling on the floor in the boys bathroom.

Correction Required: Evidence of infestation of insects or rodents in the facility is not permitted.

Noncompliance Area: During the renewal inspection the some of the tiles in the drop ceiling in the indoor gym room where not in the proper position correctly.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: DURING COMPLAINT INVESTIGATION ON 1-8-18, CERT REP DISCOVERD A LAPSE IN SUPERVISION RESULTED IN AN OPPORTUNITY FOR A PHYSICAL CONFRONTATION TO OCCUR BETWEEN TWO STUDENTS. UPON BEING SEPARATED FROM EACH OTHER, AV SUSTAINED A SCRAPE UNDER AN ARM AS A RESULT OF AP'S REMOVING AV FROM THE SITUATION.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: A hard plastic red chair in room 14 was observed cracked at the arm rest and back.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child`s size, age and special needs.

Noncompliance Area: Staff person #2 was observed sleeping room 21, preschool class, while the children napped.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times.

Noncompliance Area: A written plan identifying the means of transporting a child to emergency care and staffing provisions were not posted in room 16/17 and room 25.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.
