Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. discovered that the services provided form had not been updated/signed at anytime over the last 6 months for child#1 and child#3

Correction Required: The services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. The operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information in 3270.124(f) (relating to emergency contact information).

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. discovered that the enrolling parent/guardian home address and home phone, along with the enrolling parent/guardian work address and work phone were missing from the files of Child #3, Child #6, and Child #8.

Correction Required: Emergency contact information must include the home and work addresses and telephone numbers of the enrolling parent. A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. observed bottles in the dish washing station in the kitchen area that were not labeled.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. observed that it has been over a year since the last time they contacted local authority in writing, making them aware of their facility details, emergency plans, and contact information.

Correction Required: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the day care facility.

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. discovered that the staff file for Staff#1 did not contain State Police Clearance.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. discovered that staff#2 and staff#3 had no received an evaluation in over a year.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Noncompliance Area: During renewal inspection on 18 JULY 2019, the certification rep. observed temperatures exceeding 82 F in the top floor of the child care facility. There were no children being cared for in this space at the time, however, if there were, no means of mechanical air circulation was available to operate.

Correction Required: If the indoor temperature exceeds 82 F in a child care space, a means of mechanical air circulation must be operating.

Noncompliance Area: During the inspection on 9/28/18 the refrigerator was not clean, the bottom of the refrigerator had colored liquid in it.

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45 F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: During the inspection on 9/28/18 the file records for child #1 and 2 did not include a completed financial agreement form.

Correction Required: An agreement shall be signed by the operator and the parent.

Noncompliance Area: During the inspection on 9/28/18 the busy bees did not have emergency contact information shall be present in a child care space for children receiving care in the space.

Correction Required: When children are in the facility, emergency contact information shall be present in a child care space for children receiving care in the space.

Noncompliance Area: During the inspection on 9/28/18 the emergency plan was not posted.

Correction Required: The operator shall post the emergency plan in the facility at a conspicuous location.

Noncompliance Area: During the inspection on 9/28/18 the Bathroom did not have a hand washing sign near the sink.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/19/17, THERE WAS NO DAILY ACTIVITY SCHEDULE IN THE BASEMENT AND ON THE 2ND FLOOR.

Correction Required: The written plan of daily activities shall be posted in the group space.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/19/17, THE 2ND FLOOR AND THE BASEMENT DID NOT HAVE A WRITTEN PLAN IDENTIFYING THE MEANS OF TRANSPORTING A CHILD TO EMERGENCY CARE.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: DURING THE RENEWAL INSPECTION ON 9/19/17, STAFF 2 FILE DID NOT HAVE VERIFICATION OF TB RESULTS ON FILE.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.
