Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff Person # 1 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Staff Person # 1 may not work in a child care position at the facility.

Noncompliance Area: An exit from the facility in the infant classroom was obstructed with a bouncy chair.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Description: 12 months prior to service and every 24 months thereafter

Noncompliance Area: Facility person #1 did not have a health assessment that was conducted with in the past 24 months. Health assessment on file was dated 3/8/22.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Description: Professional development certificate

Noncompliance Area: Facility person #2 did not have their pre-service health and safety training on file during the renewal inspection.

Correction Required: Completion of professional development shall be documented by the signature and title of a representative of the professional development entity and include the date professional development was completed. Documentation shall be retained in the facility person's file or maintained in an electronic system as designated by the Department. Documentation of the completion of the professional development under subsection (f) taken from September 30, 2016, forward satisfies this requirement.

Description: Building Surfaces

Noncompliance Area: The floors in the Pre-K space were dirty.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: During the renewal inspection the school age room was inspected and there was no schedule of daily activities posted in that classroom.

Correction Required: A written plan of daily activities and routines, including a time for free play shall be established for each group. The plan shall be flexible to accommodate the needs of individual children and the dynamics of the group. The written plan of daily activities shall be posted in the group space.

Noncompliance Area: During the renewal inspection the Pre k room did not have posted in the room, emergency transportation procedures identifying a means of transporting children in the event that emergency care is needed.

Correction Required: A written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency shall be displayed conspicuously in every child care space and shall accompany a staff person who leaves on an excursion with children.

Noncompliance Area: During the renewal inspection the program's emergency plan was reviewed and was found not to contain all components required to be in the plan..

Correction Required: The facility shall have an emergency plan that includes all components as per regulation.

Noncompliance Area: During the renewal inspection the file for staff person number 1 was reviewed and it did not contain proof of completion of the mandatory child abuse reporting training.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Staff person #1 must complete mandated child abuse reporting training and in the future proof of this training must be in all staff files.

Noncompliance Area: In review of children records, child 1, an infant, failed to have updated health report completed within 6 months. Child 1 health report was last completed on 05/13/17 and child 3 health report was last completed on 12/15/17.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler

Noncompliance Area: In review of staff records, staff person 1 failed to have copy of PA state police clearance contained in file.

Correction Required: A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Correction Required: A facility person`s record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: During the allocated unannounced monitoring inspection, the cots did not have 2 feet of space on three sides of the cots while the children were napping in the preschool classroom.

Correction Required: At least 2 feet of space is required on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: During the allocated unannounced monitoring inspection, staff #1-3 did not have emergency plan training on file.

Correction Required: A facility person`s record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: During the allocated unannounced monitoring inspection, staff #2 did not have a signed disclosure on file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Noncompliance Area: During the renewal inspection in the preschool room there was a high chair missing the T-shaped safety strap.

Correction Required: High chairs shall have a wide base and a T-shaped safety strap.

Noncompliance Area: During the renewal inspection in the infant room staff #3 did not know the specific children she was responsible for supervision for.

Correction Required: Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises.

Noncompliance Area: During the renewal inspection the menu was not posted in the facilty.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: During the renewal inspection some of the bottles were not labeled with the childrens names in the infant room.

Correction Required: Disposable nursers and bottles shall be labeled with the child`s name.

Noncompliance Area: During the renewal inspection in the preschool room there were 2 staff persons with a group of 14 children, the youngest child was 2 years old.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level).

Noncompliance Area: During the renewal inspection there was hand sanitizer in the hallway on the wall going into the afterschool classroom.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
