Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.The legal entity must submit documentation in a format approved by DHS proving that the facility is maintaining the required staff:child ratios at all times. The legal entity will confirm continuing compliance by sending copies of the documentation to the SOUTHEAST Region Office on a weekly basis for a period of TWO months after DHS approves the documentation format.

Noncompliance Area: ON 1/7/20 DURING THE RENEWAL INSPECTION, THE SHELVING UNITS IN THE 2ND AND 3RD ROOMS FROM THE FRONT OF THE BUILDING WERE WOBBLY AND EASILY TIPPED OVER. THERE WAS AN EXPOSED SCREW ON ONE OF THE SHELVES.

Noncompliance Area: ON 6/12/19 DURING A COMPLAINT INVESTIGATION, THE FACILITY HAD VISIBLE MOLD ON THE WALL IN THE BATHROOM USED BY THE CHILDREN. THERE WAS A HOLE IN THE FLOOR OF THE FOOD PREP AREA AT THE ENTRANCE TO THE SAME BATHROOM. THE FLOOR WAS ALSO SOFT AROUND THE HOLE.
