Noncompliance Area: During the renewal inspection on 8/24/18, a high chair, in the Infant & Young Toddler Room, was missing part of the T-shaped safety strap.

Correction Required: High chairs shall have a wide base and a T-shaped safety strap.

Noncompliance Area: During the renewal inspection on 8/24/18, damaged paint and plaster was present on the wall and floor by the first floor exit #2 under the pull alarm.

Correction Required: Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.

Noncompliance Area: During the allocated inspection there was an infant in a pack and play in the back row in the infant room upper area sleep. The staff person could not see the infant from where she was sitting on the lower area near the entrance of the room because the railing and other pack in plays were blocking the view.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: During the allocated inspection a bottle was not labeled with the child's name in the infant room.
