Noncompliance Area: At the time of the renewal inspection on July 23, 2026, Staff Person #2 was observed supervising eight children, ranging in age from young toddlers to preschoolers, in the outdoor play area. Staff Person #1 later joined Staff Person #2; however, Staff Person #1 left the outdoor play area multiple times to clean and pick up items inside the facility, resulting in Staff Person #2 being left alone with the children.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in § 3270.51 (relating to similar age level). Two staff must be present with 8 children ranging in age from young toddlers to preschoolers.

Noncompliance Area: At the time of the renewal inspection on July 23, 2026, it was observed that the first aid kit with a bottle of water and emergency contact forms were not present with the children while they were in the outdoor play area.

Correction Required: One first-aid kit per child care group must accompany children and facility persons on excursions from the facility. Each first aid kit taken on an excursion must contain a bottle of water in addition to the items specified at §3270.75(c). When children leave the facility on walking and riding excursions, emergency contact information specific to each child on the excursion shall accompany a staff person on the excursion.

Noncompliance Area: At the time of inspection on 4/16/26, file of staff person #2 included pediatric first aid and CPR certificate from 12/11/23 which is expired. An updated certificate was not provided at the time of inspection.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: At the time of renewal inspection, file of staff persons #2, #3, #4, and #5 did not include written record that emergency plan training was conducted on an annual basis. Last emergency plan training on file of staff person #2 was from 4/1/24, on file of staff persons #4 and #5 were from 3/2/24.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.
