Noncompliance Area: During renewal inspection on 7/16/20 observed that fencing replacement on the outdoor playground was incomplete. The area between brick pillars did not have fencing/barriers and gates that restrict children from running into the road. These conditions create a potential hazard for children who would use this play space.

Correction Required: If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During unannounced monitoring inspection on 11/1/19 observed staff #1 with 9 children ranging in age from young toddlers to preschoolers in the lunch room area. Staff #2 was in toddler area separated by two half walls and was not able to see, direct and assess the activity of the supervisee. Staff #3 left lunch room area to open the door.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: During the renewal inspection on 3/12/19 observed open pillowcases full of foam with torn pieces in preschool area.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During the renewal inspection on 3/12/19 observed an outdoor equipment installed over shredded/recycled rubber with approximately 4 inch depth that didn't meet the recommendation of the United States Consumer Product Safety Commission.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: During the renewal inspection on 3/12/19 observed wood mulch with a diameter of less than 1 inch on the infant/toddler playground.

Correction Required: Pea gravel and other materials with a diameter of less than 1 inch may not be used in spaces where infants or toddlers receive care.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: During the renewal inspection on 3/12/19, a health assessment of staff person #1 didn't include an examination for communicable diseases and the results of that examination as well as information on medical problems that might threated the health of the children or prohibit a staff person from providing adequate care to children. Noncompliance Area: During the renewal inspection on 3/12/19, file of staff person #3 contained a copy of child abuse clearance from 1/31/14 that was over 60 months old.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #3 may not work in a child care position at the facility.

Noncompliance Area: During the renewal inspection on 3/12/19 observed in preschool area 13 children ranging in age from young toddler to preschool with only two staff persons present.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). Three staff persons would be required for a group of 1 young toddler, 4 older toddlers and 8 preschoolers.

Noncompliance Area: During the renewal inspection on 2/12/19, hot water temperature in girls bathroom used by children was measured at 120F.

Correction Required: Hot water temperature, in areas accessible to children, may not exceed 110 F.

Noncompliance Area: During the renewal inspection on 3/12/19 observed a torn frayed carpet near TV in a school age area that is a tripping hazard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.
