Noncompliance Area: CHILD 1'S HEALTH REPORT DID NOT STATE IF THE CHILD WAS FREE FROM COMMUNICABLE DISEASE AND ABLE TO PARTICIPATE IN CHILD CARE.

Correction Required: A health report shall include a statement that the child is able to participate in child care and appears to be free from contagious or communicable disease.

Noncompliance Area: CHILD 1 AND 2 DID NOT HAVE PROOF OF A CURRENT FLU IMMUNIZATION ON FILE (CHILD 1 LASTED DATED 2/14/2018; CHILD 2 DID NOT HAVE PROOF OF ANY FLU IMMUNIZATION).

Correction Required: The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: STAFF 3 AND 6 DID NOT HAVE PROOF OF A CURRENT HEALTH REPORT ON FILE (STAFF 3 LASTED DATED 3/28/17; STAFF 6 LAST DATED 7/14/19).

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.

Noncompliance Area: STAFF 1 AND 7 DID NOT HAVE PROOF OF THE RESULTS OF A TB TEST ON FILE.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: STAFF 4 WAS MISSING THEIR NSOR (HIRED 11/11/19); AND STAFF 7 HAD A FBI CLEARANCE THAT WAS EXPIRED (DATED 5/27/14); STAFF 3 DID NOT HAVE PROOF OF ANY MANDATED REPORTER TRAINING (HIRED 7/1/19); STAFF 1'S MANDATED TRAINING EXPIRED (DATED 1/19/15).

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 4 AND 7 -- may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person # 4 AND 7--- may not work in a child care position at the facility.

Correction Required: A director is responsible for written evaluation of staff persons on a regular basis, a minimum of one evaluation every 12 months.

Correction Required: The operator shall require the parent of an enrolled child, including a child, a foster child and a relative of an operator or a facility person, to provide an initial health report no later than 60 days following the first day of attendance at the facility.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: At the time of the inspection the provider did not produce the current copy of the insurance policy shall be on file at the facility.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.

Noncompliance Area: At the time of the inspection the record of child #1 did not have the required 6 month updates on financial agreement or emergency contact form.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: At the time of the inspection the records of children #2 and #3 did not have current valid health assessments.

Correction Required: A child's record shall contain initial and subsequent health reports.
