Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed a bookshelf/cubby on the bottom floor of the facility was not anchored to the wall.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During a compliant investigation conducted on 2/1/2022, the Certification Representative observed that the facility did not have at least 2 feet of space on three sides of a bed, cot, crib or other rest equipment while the equipment is in use.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed that the infant room refrigerator did not have an operating thermometer .

Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, 5 staff files were reviewed. Staff member # 1 file did not contain a mandated reporter training certificate. Staff member 4 file did not contain a FBI, PA child abuse, mandated reporter training clearances. Staff member # 5 did not contain a FBI clearance and a mandated reporter training certificate. The provisional waiver for new hire does not apply for these staff member as they are past their 45 days of their hire date. Staff member # 1,4 and 5 cannot work in a child care facility.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). LACKING REQUIRED HIRING DOCUMENTS: Facility Person # 1,4 and 5 may not work in a child care position at the facility. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed an electrical outlet in the upstairs child care space did not have a protective cover in it . The space was being used by 6 - 2 year old children.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed a bottle of bleach and disinfectant were left on the floor of the upstairs bathroom.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed an unmarked bottle with liquid was left in the upstairs bathroom.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be stored away from food, food preparation areas and child care spaces.

Noncompliance Area: During a complaint investigation conducted on 2/1/2022, the Certification Representative observed a bookshelf/cubby on the bottom floor of the facility was not anchored to the wall.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: During a renewal inspection conducted on 11/9/2021, 2 staff files were reviewed. Staff member #1 file did not contain verification of health and safety training.

Correction Required: Training conducted by an entity that is licensed or certified professionally competent in the training topic is acceptable and may count toward the annual requirement for 12 clock hours of child care training.

Noncompliance Area: During a renewal inspection conducted on 11/16/2021, 2 staff files were reviewed. Staff member #2 file contained a fire safety training certificate that expired on 4/30/2021. The fire safety training in not valid.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Correction Required: A primary shall have attained one of the following qualification levels specified at 3280.34(b)(1) - (5).A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Noncompliance Area: During the initial inspection on 1/30/19, spaces/gaps in the chain link fence were observed that would allow a child to squeeze through. A wooden fence on the right in the outdoor play space also had a gap/space posing an unsafe condition for a child prior to the issuance of the initial certificate of compliance.

Correction Required: Outdoor or indoor play space shall be safe for large muscle activity. If unsafe areas or conditions are in or near an outdoor play space, fencing or natural barriers are required to restrict children from those unsafe areas or conditions.

Noncompliance Area: During the initial inspection on 1/30/19, the indoor temperature of the facility measured for 59 F prior to the issuance of the initial certificate of compliance.

Correction Required: The indoor temperature shall be at least 65 F.

Noncompliance Area: During the initial inspection on 1/30/19, the facility did not have an operable telephone prior to the issuance of the initial certificate of compliance.

Correction Required: A facility shall have an operable telephone. A facility that has an unpublished telephone number shall make the number available to the parent, the release persons of the children in care, an agency that oversees or funds the facility and the appropriate regional office of the Department.

Noncompliance Area: During the initial inspection on 1/30/19, the glass entrance doorway did not have visual identification prior to the issuance of the initial certificate of compliance.

Correction Required: A visual strip or other visual identification shall be placed on glass located in a traffic area, a child care space or a play space.
