STAFF PERSON # 1 LEFT THE SPACE LEAVING THE CHILDREN ALONE TO OPEN THE DOOR.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises.

Noncompliance Area: ON DECEMBER 17, 2019 DURING THE UNANNOUNCED INSPECTION, THE CERTIFICATION REPRESENTATIVE OBSERVED CHILD # 1 ASLEEP INSIDE OF A CRIB THAT DID NOT HAVE AT LEAST 2 FEET OF SPACE ON THREE SIDES.

Correction Required: A parent is required to review and update the emergency contact information and the financial agreement at least once in a 6-month period or as soon as there is a change in the information.

Noncompliance Area: Certification Rep has confirmed that Medical assessments presented during renewal inspection were not completed and signed by the physician listed. Staff # 1 and Staff #2 were never patients of physician listed on the medical assessments.

Correction Required: The Department may deny, refuse to renew or revoke a certificate of compliance for fraud or deceit in obtaining or attempting to obtain a certificate of compliance.

Noncompliance Area: There were either multiple fields missing or there was no Agreement form for Child # 1,#2, #3, #4 and #5 Agreement forms at the time of the renewal inspection.

Correction Required: An agreement shall be signed by the operator and the parent.An agreement shall specify the amount of the fee to be charged per day or per week.

Noncompliance Area: There were either multiple fields on the Emergency Contact Form or no State approved Emergency Contact form available for Child # 2,#3, #4 or #5 at the time of the Renewal inspection.

Correction Required: Emergency contact information is required for each enrolled child. Emergency contact information must reference who shall be contacted in an emergency.

Noncompliance Area: Policy number and name of insurance were missing from Emergency Contact Form for Child #1 at the time of the renewal inspection.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Noncompliance Area: There was a 5 month old child that was changed by a staff and their hands were not washed after the diapering during the renewal inspection.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: Review of files reveal that Child #2, #3 , #4 and #5 did not have the consents for minor first aid or ER care at the time of the renewal inspection.

Correction Required: A child's record shall contain signed parental consent for emergency medical care for the child. Correction Required: A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: Review of files reveal that Staff #3 had a Volunteer Child Abuse report as opposed to a child abuse clearance for employment purposes. Staff #4 did not have Mandated Reporter Training at the time of the renewal inspection.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. LACKING REQUIRED HIRING DOCUMENTS: Facility Person # -- may not work in a child care position at the facility. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #--- may not work in a child care position at the facility.

Noncompliance Area: There was no fire drill documentation on site at the time of the renewal inspection.

Correction Required: A fire drill shall be held at least every 60 days. Facility persons and children in attendance shall participate in the fire drill. Facility persons and children shall exit the building, weather permitting.

Noncompliance Area: During the Initial Inspection, there was an apparatus that was not fully assembled. There was one appratus that was assembled in the exterior playspace with a fall height and there was no protective surface covering to comply with the fall height. There was also an Easel but there were no markers; crayons; clay or pencils to promote fine motor skills.

Correction Required: Play equipment and materials shall include toys to handle and manipulate and art materials for tactile development. Play equipment and materials shall include toys and equipment for large muscle development.

Noncompliance Area: It was later determined after leaving the facility for the initial inspection that the assigned Director had two other programs that they would be operating in addition to this facility which would be a third.

Correction Required: A director shall be employed by a facility and be present at the facility site a minimum of 30 hours per week.

Noncompliance Area: During the Initial Inspection it was observed that there were several bags of trash; a door and other material; along with some play equipment that had not been assembled as well as a pipe that posed as a tripping hazard in the area. Lastly, the ground had multiple surfaces that had craters or dents and the concrete was not smooth.

Correction Required: Outdoor or indoor play space shall be safe for large muscle activity.

Noncompliance Area: The First Aid Kit was missing soap at the time of the Renewal Inspection.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.

Noncompliance Area: The handwashing sign that was displayed in both bathrooms did not state that a facility person and an able child shall wash his hands after toileting and before eating.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.
