Noncompliance Area: Facility staff # 1 had pediatric first aid and cpr that expired on 3/15/26.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

Noncompliance Area: Operator has not conducted a fire drill in over 60 days since 1/14/26.

Correction Required: Fire drills are conducted at least once every 60 days.

Noncompliance Area: Facility fire alarms have not been tested in over 30 days since 1/14/26.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Description: Clean and good repair/Small Toys and Objects

Noncompliance Area: Facility childcare space has scissors on a shelf accessible to toddlers and infants. Facility kitchen sink has a sharp knife in it accessible to the children when entering through the kitchen to use the childcare bathroom. Facility outdoor play climber has a broken steering wheel in the yard outside used by the childcare children.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts. Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Description: Services provided

Noncompliance Area: Child # 2 has not have a child service report completed in over 6 months since 4/22/24.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: Child #3 & # 4 do not have their fee agreements signed by the parent or operator and they do not have their dates of admission on file.

Correction Required: An agreement shall be signed by the operator and the parent. An agreement shall specify the date of the child's admission.

Description: Insurance coverage information/Name/address/phone release person

Noncompliance Area: Child # 3 & # 4 do not have health insurance policy coverage information on file and they do not have release persons information on file on their fee agreements.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable. Emergency contact information must include the name, address and telephone number of the individual designated by the parent to whom the child may be released.

Description: Name on bottle/Original label

Noncompliance Area: Facility has A&D ointment for a child that is not labeled and expired in November 2022.

Correction Required: The label of a medication container shall identify the name of the medication and the name of the child for whom the medication is intended. Medication shall be administered to only the child whose name appears on the container. Medication shall be stored in accordance with the manufacturer's or health professional's instructions on the original label.

Description: Disposable diapers

Noncompliance Area: Facility's Dehour diapering can is not hands free

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

Description: Wholesome

Noncompliance Area: There is used/cut, uncovered, green peppers and avacado in the refrigerator that was previously served to the childcare children.

Correction Required: Food stored, prepared or served shall be clean, wholesome, free from spoilage, free from adulteration and safe for human consumption.

Description: Consent for administration of minor first-aid required prior to admission

Noncompliance Area: Child # 4 does not have signed consent for minor first aid on file.

Correction Required: A child's record must contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Regulation: 3290.24(d)

Description: Emergency plan

Noncompliance Area: The facility has not conducted an emergency drill in over 12 months.

Correction Required: Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.
