Noncompliance Area: During the renewal inspection, staff 1 was alone with the children when certification representative arrived. Staff 1 was alone with them for approximately 15 minutes. Upon file review, it was observed that staff 1 did not have pediatric first aid/cpr training.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Children 1 and 3 did not have permission for emergency medical care and minor first aid.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Description: Availability of certificate of compliance and applicable regulations

Noncompliance Area: It was observed that the regulations were not electronically available to parents. Instructions on how to contact the regional office also were not made available to parents.

Correction Required: The operator shall provide the parent of each child enrolled in the facility with information on how to access the regulations in this chapter electronically and with instructions for contacting the appropriate regional child care office.

Regulation: 3290.173(a)

Description: Age appropriate restraints

Noncompliance Area: During the allocated unannounced inspection on 3/13/2026, it was observed that staff 1 was transporting a child. Through a file review, it was found that the child was 1 years old and under 40 pounds. In assessing the car seat in the vehicle, it was found that it was forward facing. According to 75 Pa.C.S. §4581 a child under 2 needs to be in a rear facing car seat until they exceed the manufacturer's height and weight. The manufacturer's documented weight on the car seat was that the child had to be rear facing if between the weight of 5-40 pounds.

Correction Required: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. §4581 (relating to restraint systems).

Regulation: 3290.95(a)/3290.95(b)

Description: System in compliance/Operator or designated staff responsibility

Noncompliance Area: During the allocated unannounced inspection on 3/13/2026, fire alarm testing was last documented to occur on 2/10/2026, making it more than 30 days since the last test.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Description: Comply with CPSL/CPSL information

Noncompliance Area: During the initial inspection on 12/3/2025, it was discovered that household members 1 and 2 also live at the facility. Household member 1 did not have a child abuse clearance on file during the time of the initial inspection. Household member 2 did not have a child abuse or state police clearance on file during the initial inspection.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Household members 1 and 2 may not be in the childcare area until all required clearances are obtained. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Household members 1 and 2 may not be in the childcare area until all required clearances are obtained. TIERED LIS: 1) Facility will not provide childcare until clearances are obtained. This portion of the plan will have an immediate correction date. 2) Facility will obtain clearances for all household members. The operator will provide a date on when the clearances will be obtained.

Description: Evacuation routes posted

Noncompliance Area: During the initial inspection on 12/3/2025, it was discovered that the facility did not have evacuation routes in the basement, 2nd floor and 3rd floor.

Correction Required: Evacuation routes are posted in a conspicuous location on each floor of the facility.
