Correction Required: A facility shall have an operable, clean refrigerator used to store potentially hazardous foods. The refrigerator shall be capable of maintaining food at 45° F or below. An operating thermometer shall be placed in the refrigerator.

Description: Services provided

Noncompliance Area: During the renewal inspection on 6/5/2026, children 1,2,3 and 4 had either no child service report on file or an outdated one. Child 1 last had a child service report completed on 6/25/2025. Child 2 last had a child service report completed on 10/27/2025. Both were last completed more than 6 months ago. Children 3 and 4 did not have one on file. Please see LIS for dates of admission and dates of birth.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: Staff 2 only used hand sanitizer on child's hand after diapering.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: Child 3 did not have consent for emergency medical care on file.

Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Locked or inaccessible - in original container/Locked

Noncompliance Area: Lysol cleaner was next to the diaper changing table. It was accessible to the children in care, as it was in the childcare area and within a child's reach. Medication, which was in a child proof medication container, was also observed to be in the cupholder in the front seat of the van. The medication belonged to staff 2. The van is used to transport children. When children would be exiting or entering the van, the medication would be accessible to them. The medication belonged to staff 2.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces. Medication shall be stored in a locked area of the facility or in an area that is out of the reach of children.

Description: Individual Records

Noncompliance Area: When certification representative arrived to the facility on 2/13/2026 to conduct an unannounced inspection, it was observed that staff 1 and 2 were working without a file.

Correction Required: An individual record is required for each facility person.

Description: Comply with CPSL/CPSL information

Noncompliance Area: When certification representative arrived to the facility to conduct an unannounced inspection, it was observed that staff 1 and household member 2 did not have all clearances on file. Staff 1 had the child abuse and FBI clearance on file but not the NSOR or state police. Household member 2 did not have any clearances on file. Staff 1 also did not have disclosure statements on file.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). Facility Person #1 and household member 2 may not work in a childcare position at the facility until all required clearances are obtained. Household member 2 will not be in childcare area until clearances are obtained. A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee. A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person #1 and household member 2 may not work in a childcare position at the facility until all required clearances are obtained. TIERED IS: 1. Staff 1 and household member may not work in a childcare position until all clearances. Household member 2 will not be in the childcare area. This portion of the plan shall have an immediate correction date. 2. If staff 1 or household member 2 will be hired in the future, all clearances will be obtained prior to staff 1 or household member 2 working for the facility. The new staff file for staff 1 and household member 2 will be sent to the regional office to be reviewed prior to the individual working with children. This portion of the plan shall have an immediate correction date.

Description: Clean and good repair

Noncompliance Area: It was observed that the outside play area had exposed bolts on the fence.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Clean, age appropriate

Noncompliance Area: It was observed that there were unlabeled cots. As per staff 1, children do rest on them.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program.

Noncompliance Area: There was no daily activity schedule posted in the group area.

Correction Required: The written plan of daily activities and routines shall be posted in a traffic area used by parents.

Noncompliance Area: Child 1 had more than 6 months between health assessments. The initial health assessment was dated 8/23/2024 and the follow up health assessment was dated 4/24/2025, making it more than 6 months between updates.

Correction Required: The operator shall require the parent to provide an updated health report at least every 6 months for an infant or young toddler.

Noncompliance Area: It was observed during the annual inspection on 6/25/2025 that the infant bottles were not labeled.
