Correction Required: A child's record must contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Noncompliance Area: During the renewal inspection on 3/18/2026, it was observed that the smoke detector on the first floor was not interconnected with the smoke detector in the basement, second floor and third floor.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Description: Infant sleep position

Noncompliance Area: During an allocated unannounced visit, it was observed that there was a toy in the pack and play of a 10-month-old baby. The baby was sleeping. Having a toy in the pack n play of a sleeping infant goes against the recommendations of the American Academy of Pediatrics.

Correction Required: Infants shall be placed in the sleeping position recommended by the American Academy of Pediatrics unless there is a medical reason an infant should not sleep in this position. The medical reason shall be documented in a statement signed by a physician, physician's assistant or CRNP and placed in the child's record at the facility.

Description: Emergency plan

Noncompliance Area: During an allocated unannounced visit on 5/2/2025, tt was observed that the emergency plan was in a binder and not posted in a conspicuous location.

Correction Required: The emergency plan shall be posted in the facility at a conspicuous location.

Description: Locked or inaccessible - in original container

Noncompliance Area: During the allocated unannounced inspection, it was observed that lysol cleaner was on the floor of the bathroom. The bathroom is used by the daycare children.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Description: Evacuation routes posted

Noncompliance Area: The facility did not have an evacuation route map posted in the basement of the facility.

Correction Required: The operator or designated staff person who is responsible for compliance with this chapter shall conduct fire drills and ensure that evacuation routes are posted in a conspicuous location on each floor of the facility.
