Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the parent and the operator. The rest equipment must be labeled with the child's name and used only by the named child while enrolled in the program.

Noncompliance Area: During the renewal inspection on 5/20/2026, staff 1's cat did not have updated rabies vaccines on file. The rabies vaccine on file was from 2024 and the paperwork stated that it would expire in 1 year.

Correction Required: A veterinarian's certificate of current rabies immunization is required for a cat or dog at the facility. The certificate shall be on file when the cat or dog is present.

Noncompliance Area: Children 1 and 2 did not have an updated children service report on file. Please see LIS for dates of admission and dates of birth.

Correction Required: An agreement shall specify the services to be provided to the family and the child, including the Department's approved form to provide information to the family about the child's growth and development in the context of the services being provided. the operator shall complete and update the form and provide a copy to the family in accordance with the updates regarding emergency contact information at § 3290.124(e).

Noncompliance Area: Child 2 did not have health insurance information documented on the emergency form.

Correction Required: Emergency contact information must include health insurance coverage and policy number for a child under a family policy or Medical Assistance benefits, if applicable.

Description: Updated toddler/preschool health report: every 12 months

Noncompliance Area: Child 1, who is a preschool age child, did not have an updated health assessment on file. The health assessment was last updated on 5/8/2024. Please see LIS for date of birth and date of admission.

Correction Required: The operator shall require the parent to provide an updated health report at least every 12 months for an older toddler or preschool child.

Description: Age and training/Age and training

Noncompliance Area: During the annual renewal inspection on 5/20/2026, staff person 1 has not completed Pediatric first aid and CPR training on or before expiration of the most current certification. This is evidenced by the previously documented Pediatric first aid and CPR training on file expiring 2/19/2026. Documentation of updated Pediatric first aid and CPR training through an approved PQAS trainer and curriculum is not on file. Staff person 1 has completed Pediatric first aid and CPR training, however the training was not completed through a Pennsylvania Quality Assurance System (PQAS) approved curriculum and the trainer has not provided documentation that they are PQAS-certified in that training curriculum. The organization was the National CPR foundation.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and CPR. All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development within 90 days of hire as listed in subsections (g)1-10. Until such time as the required training has been completed, staff person 1 must be supervised, when interacting with children at a minimum by, a family childcare home staff person who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff person(s) available to supervise staff person 1, staff person 1 may not work in a child-care position at the facility.

Regulation: 3290.64(a)

Description: Locked or inaccessible - in original container

Noncompliance Area: In the kitchen, toxics were accessible to children in cabinets that were unlocked. Toxics included various cleaners. The kitchen is accessible to the children when accessing the bathroom used by the daycare.

Correction Required: Cleaning materials and other toxic materials shall be stored in an original labeled container or in a container that specifies the content. Toxics shall be kept in a locked area or in an area inaccessible to children, and shall be stored away from food, food preparation areas and child care spaces.

Regulation: 3290.94(a)(1)

Description: Every 60 days

Noncompliance Area: During the renewal inspection on 5/20/2026, it was observed that the last fire drill was documented on May 9th, 2025 making it more than 60 days since the last fire drill. Staff 1 states that she lost the documentation that had the updated fire drills on them.

Correction Required: Fire drills are conducted at least once every 60 days.

Regulation: 3290.95(a)/3290.95(b)

Description: System in compliance/Operator or designated staff responsibility

Noncompliance Area: During the renewal inspection on 5/20/2026, it was observed that the smoke detector on the third floor was not interconnected with the other smoke detectors. It was also observed that the last fire alarm test was May 9th, 2025 making it more than 30 days since the last fire alarm test.

Correction Required: Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)). The operator or designated staff person who is responsible for compliance with this chapter shall ensure the requirements in subsection (a) are met.

Noncompliance Area: There were two pack n' plays in the childcare space. Neither one was labeled with a child's name. As per provider, one child will occasionally use one.

Description: Emergency plan/Emergency plan

Noncompliance Area: The last update to the emergency plan that was on file was from March 15th, 2020. There was no emergency plan drill on file.

Correction Required: The operator shall review the emergency plan at least annually and update the plan as needed. Each review and update of the emergency plan shall be documented in writing and kept on file at the facility. Emergency drills shall be conducted annually. Annual emergency drills shall be documented and on file at the facility.

Description: Age and training

Noncompliance Area: Staff 1 did not have 12 hours of annual childcare training on file. Staff 1 only had 7.5 hours on file.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

Description: Requirements

Noncompliance Area: As per provider, she would take the children outside in the backyard for the children to get air sometimes. The backyard had broken wooden boards from the wooden fence throughout the yard.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play area, shall be kept clean, in good repair and free from visible hazards.
