Today as I drove passed the facility at 3:05pm it was noted that one teacher was sitting in a yellow chair on the stage area while children were playing in other areas of the playground. Ms. Potts was observed standing in front of the teacher talking. As I entered the parking lot area a few minutes later teachers were observed huddled up underneath the shade tree and not positioned in all areas to adequately supervise the children. Once teachers noticed I was there they separated and positioned themselves throughout the playground area | |||

As we pulled into the parking lot to the faclity it was observed that one (1) teacher (LD) was sitting on the stage area and looking at her phone and not engaging with children. As we walked to the playground area she was still looking down at her phone. Once she was aware of our presence she got up and started moving about the playground area. | |||

| Children were not adequately supervised at all times. Five (5) teachers were at the far end of the playground sitting and standing for five (5) minutes. There were two (2) children sitting by them. For the five (5) minutes not one of the teachers turned to check on the twelve (12) children on the far side of the playground. This playground is extremely long and needs constant zoning by teachers. | |||

Upon my arrival to the faclity at 9:48 am the group of children ages two years of age and the group of children three through five years of age were observed playing on the playground area closest to the parking lot. Teachers were observed sitting together as a group near the sand box and the black barrier near the swings while children were playing in other areas of the playground not near the teachers.
