| Children were not adequately supervised at all times. Today, I observed two (2) substitute staff members seated at the outdoor tables beneath the shade canopy while children were playing in various areas of the playground. One (1) child was on the back side of the climber, one (1) child was at the swings, and one (1) child was in the green half-egg–shaped sit-and-spin. Additionally, two (2) children in wheelchairs were positioned beside the two staff members. | |||

| Children were not adequately supervised at all times. On 10/20/2025 a child was observed on the slide outside eating some type of fecal matter that was on the playground. Staff were unsure if it was from an animal or human but several mounds of unknown animal excrement was located on the playground along the fence line under some leaves. Staff members did not see where the excrement came from, how he got, and how long he had. | |||
