| Children were not adequately supervised at all times. The administrator reported on January 30th, 2023, three (3) staff members were outside on the playground with seventeen (17) children ages three-through-five years old. One (1) teacher was involved in a music activity with the children, another teacher was getting children to line up to do a head count and the third teacher was getting ready to leave for the day when one staff noticed that two (2) children were coming out from behind the climbing wall. After the administrator became aware of the situation and received more details, two children reportedly were inappropriately touching each other without the staff's knowledge.
