The breakfast served to children was biscuits, sausage, applesauce, and milk. The substitutions were not recorded on the menu prior to serving the food to children.

The teacher’s aide that is seventeen (17) years of age and was left alone with children was not under the direct supervision of a staff member who is credentialed with the North Carolina Early Childhood Credential or equivalent.

One (1) staff, a teacher’s aide, that is seventeen (17) years of age was left alone with children, left in charge of children, and counted in staff/child ratio during the visit while the lead teacher was in the office for approximately 10 minutes. Later, the lead teacher entered the kitchen area at approximately 11:15am until approximately 11:25am to get the children's lunches, which left the teacher's aide in charge of children and counted in staff/child ratio.

A child was injured on 2/9/24, 911 was called, and the parent took the child to the doctor.
