The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. Outside on the playground areas the toys were covered with mud, mold and sand. On the larger playground there was a broken board on the wooden picnic table. The outdoor area the older childnre use to access the playground area has overgrown weeds, broken toys and a dirty storage bin filled with dirty toys.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the classroom for children ages one year of age there were two (2) electrical outlets left uncovered below the shelf closet to the two year old room.

In the classroom for children one year of age there were batteries, disinfectant wipes and razor blades stored on an open shelf closest to the entrance to the two year old room.
