There was one staff file reviewed. The fingerprint clearance card for the staff member was verified as valid via the DPS website. Please send documentation of restroom exhaust operation, as well as copies of the fire inspection and liability insurance along with the plan of correction within 10 days of receipt of this Statement of Deficiencies. The following was discussed, but not limited to: *Moving all facility equipment out of the reach of children and cleaning of the activity areas prior to the start of care.
