Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 08-19-2025, and are subject to changes pending programmatic review. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Please send to the Department a copy of the current fire inspection report. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. Emergency card documentation. 2. Playground maintenance. 3. Classroom maintenance. 4. Bathroom maintenance. 5. Medications.

Initial Comments: The following deficiencies were observed during the annual compliance inspection conducted on August 22, 2024, and are subject to changes pending programmatic review. Compliance Officer: Celeste Angulo 2 of the 2 fingerprint clearance cards reviewed were verified through the DPS website during the inspection. Please complete the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies. A link to the Empower Survey was emailed to the facility director. The following items were discussed but are not limited to: *Completion of the Criminal History Affidavit form needs to be completed before the date of employment *Food menu needs to have specifics *Accident and Emergency procedures need to be updated every 12 months
