A Staff stated Staff 2 was from a different location, and did not bring a copy of their staff file. *It was noted the ADCS affidavit was not available for review in the 11 staff files reviewed. The Surveyor asked 2 staff members for the DCS affidavits, but the staff members did not know where they were located.

Staff 2's file was unavailable for review during the inspection, therefore Staff 2's education and child care experience were unable to be verified. *At the time of the inspection, Staff 3 was observed working alone supervising the children in the 4's room. After a review of Staff 3's file, it was determined Staff 3 was lacking documentation of education to qualify as a teacher-caregiver. *At the time of the inspection, Staff 4 was observed working alone supervising the children in the school age room. After a review of Staff 4's file, it was determined Staff 4 was lacking documentation of the required 6 months of previous childcare experience to qualify as a teacher-caregiver. *At the time of the inspection, Staff 5 was observed working alone supervising the children in the 1's room. After a review of Staff 5's file, it was determined Staff 5 was lacking documentation of education to qualify as a teacher-caregiver. A Staff stated Staff 2 was from a different site and did have a copy of their staff file on facility premises.
