Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/04/2025, and the inspection is subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the Provider at the beginning of the inspection. Please submit the plan of correction and upload the Certificate of Liability, Fire Inspection, and the Food Establishment (Sanitation) Permit in the LMS Portal within 10 days of receiving the statement of deficiencies. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. The Emergency Disaster Contact Form and invitation link to complete the Empower Self Assessment were emailed to the Provider. During the exit interview, the following items were discussed, but are not limited to: *The specific foods offered to children will be included on the menu *Diaper wipes will be labeled
