The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility representative at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please email pictures of the new diaper changing areas in the 2 rooms that are located in the toilet room. 1 of 1 Fingerprint Clearance card reviewed was valid via the DPS website search. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Children's Records must be complete. 2. Missing middle section of door to toilet room. 3, Children's faces must not be covered at naptime. | |||

A full inspection was conducted. 20 of 20 Fingerprint Clearance Cards reviewed were verified as valid through the DPS website. The following was discussed, but not limited to: *All pages of the Criminal History Affidavit are included. *Employees sign all required documents. *Employees provide proper qualifying documentation. *Vehicles are maintained in a safe condition. *All exits are accessible during hours of operation. *Emergency, Information, and Immunization Record cards have all required information. *The facility must be maintained free from hazards. Please submit the following: * Written plan of corrections via the licensing portal, * Photos/documentation of corrections. NOTE: Licensure is pending the above requirements. | |||
