Findings: Based on Surveyor's observation and a discussion with the staff in the 3 year olds room, the lid to the trash can that contained food waste (fruit, bread and milk) was missing. The staff stated the lid had broken recently and the trash can no longer had a lid.

Findings: Based on the Surveyor's observation and a discussion with the Director, a 3 foot wide by 3 foot deep hole was observed in the wood chips near the side walk area in the front playground. The Director stated the hole was from a repair that had been recently completed to repair a leaking water pipe.

Findings: Per Allegation #1- Based on the Surveyor's review of the Parent Communication Forms for the 2 year old room for the months of April and May 2021 regarding incidents of biting and on interview with the Director and the 2 year old room teacher, a staff member failed to supervise each enrolled child at all times. There were 17 documented biting incidents in the 2 year old room between the months of April and May 2021. In several incidents, the teacher and Director acknowledged that the bites were severe enough to break the skin. The teacher stated one of the bites left a permanent mark on a child. In another incident, a child was bitten by another child as the child was having their diaper changed. In another incident, the teacher stated she was talking to a child about not biting other children after the child had just bitten a child. As she was talking to this child, the child turned and bit a second child.
