Based on documentation, Staff #3 had completed 11 hours during the review period.

Findings: Surveyor reviewed 4 staff files and interview with the Director determined that 1 of the 4 staff files lacked documentation verifying that results of a negative Mantoux skin test given to a staff member was within 12 months before the starting date of employment. * Staff #1-starting date of employment-4/4/2018. The file had documentation indicating that a negative Mantoux skin test was read on 8/26/2015. The results were verified before 12 months prior to the starting date of employment.

Findings: During an inspection of the facility, Surveyor observed the following items: 1) The ceiling vent had a white fluffy substance on the grill area in the bathroom in Room #103 and #104; 2) The green vinyl floor mat in the library area was worn and ripped along edges in Room #103.
