| Initial Comments: No deficiencies were observed at the time of the Initial Compliance Inspection conducted on 03/10/2026 and is subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 12 of 12 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring the outside play equipment is cleaned before children play with/on it. 2). Ensuring that staff files are completed and updated. 3). Ensuring that fire drills are completed monthly. 4). Ensuring that medication consent forms are fully completed. 5). Ensuring that suspected child abuse is reported to DHS and other agencies. 6). Ensuring written approval is received from parents for the use of personal products. 7). Ensuring DHS is made aware of any closures at the facility. 8). Ensuring DHS has immediate access to staff and children for interviews during complaint investigations. | |||
