A complete inspection of the facility was not conducted. The Plan of Corrections must be submitted using the AZDHS Licensing Portal within ten (10) days from the date the Statement of Deficiencies is received. The following staff-to-children ratios were observed: Ones 2:9 Twos 1:8 Threes and up 2:21 1 staff member was interviewed. The Maricopa County Sherriff's Office Incident Report was reviewed. Upon completion of the Complaint Investigation (Case Number 00086299), it was determined from the review of the Maricopa County Sherriff's Office Incident Report, the staff interview, and the Compliance Officers' observations that 2 of 2 allegations lacked sufficient evidence to be substantiated.
