|Findings: Based on observation and interview with the Director, the Surveyor determined that the West Room lacked a dated lesson plan for the class for the current week.|
|Findings: During an inspection of the facility, Surveyor observed and determined that the facility first aid kit lacked Antiseptic.|
|Findings: Based on Surveyor observation, Surveyor determined the following hazard on the outdoor playground: 1) There was a blue picnic table with several areas of chipping paint.|
|Findings: Based on Surveyor observation and discussion with the Director, it was found that the depth of wood chips under the slides and climbing equipment on the outdoor playground was 3 inches.|
|Findings: After a review of 6 children's Emergency, Information and Immunization Record cards, it was determined the following were lacking: Child 1: second emergency contact name and phone number. Child 2: father's information;. Child 3: second emergency contact name and phone number.|
|Findings: After a review of two staff files, it was determined the following was lacking documentation of the 10 day new staff training, as required: Staff 1 (start date 8/28/2018)|
