|Findings: Based on surveyor's review and an interview with the director, it was found that child 1 was lacking the back of the card which contains any medical, nutritional information as well as a parent's signature and date. Child 2 was lacking the name of the person to be called first in an emergency. Child 3 was lacking any health care provider information.|
|Findings: Based on surveyor's review and an interview with the director, it was found that staff 2 began employment 8/5/20 and staff 3 began employment 8/5/20 and both staff were lacking a copy of the back of their fingerprint clearance cards.|
|Findings: Based on surveyor's observation, it was found that in room 4, there was a radio next to the diaper changing table and diaper sink. Also, the diaper changing surface had straps attached to the the pulldown table.|

|Findings: Based on surveyor's observation, it was found that a staff was disposing an open pail of soiled diapers and pullups. Surveyor observed that there was no diaper changing area in the room with 2 yr old children.|

|Findings: Based on surveyor's observation, it was found that the ceiling exhaust fans in both 3-4's bathrooms were dirty with a dust buildup.|

|Findings: The Surveyor found a copy of the back of the fingerprint clearance cards for Staff 1 and Staff 2 were not in the staff file.|
