Word of Life Preschool is a Christ-centered preschool in Surprise, AZ. Our friendly staff is committed to enabling our students to achieve their God-given potential through spiritual development and academic growth. We offer full day classes and extended care as well as part time classes. Our class sizes are small which allows us to provide our students with individualized instruction and maximize their academic experience.

Initial Comments: The following deficiency was observed at the time of the Complaint Investigation (Case 00144213 and 00145233) conducted on 10/16/2025, and are subject to changes pending programmatic review. A complete inspection of the facility was not conducted. Submit the Plan of Corrections using the AZDHS Licensing Portal within ten (10) days from the date the Statement of Deficiencies is received. The following staff-to-children ratios were observed: Ones/Twos: 2:10 Threes: 2:9 Threes/Fours: 1:9 Fours/Fives: 1:12 The Department contacted the complainant, who did not respond, on 09/08/2025 and 09/17/2025. 4 staff members and the Director were interviewed Documentation reviewed: Classroom Rosters for 08/11/2025-08/22/2025. Upon completion of the Complaint Investigation (Case 00144213 and 00145233), it was determined from staff/Director interviews, facility documentation and the Compliance Officer's observations two allegations were substantiated. The third allegation lacked sufficient evidence and was unable to be substantiated.

The Notice of Inspection Rights were provided to the licensee at the time of the inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection.

Initial Comments: There were no deficiencies found during the Compliance Inspection conducted on 08/28/2024. The Empower Self-Evaluation survey was emailed to the licensee at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. The Compliance Officer is Tara Farrell.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 08/31/2023, and are subject to changes pending programmatic review.
