Children were not adequately supervised at all times. During the visit, the owner left six preschool children unattended in space 4 when she walked out of the room and down the hall. In the second instance, the owner left six preschool children who were eating lunch in space 4 unattended when she left the classroom.

Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Three of Ms. Donaldson's trainings were more than five years old, including topics on infectious disease, medication and hazardous materials.

When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. There were only two of three toys available in the book/language areas of the preschool and up classrooms. There was one item accessible to children in the art area in the two-year-old classroom. This was corrected when Ms. Donaldson and the teachers added materials to the activity centers.

Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. Special diet information was posted only in one of five classrooms. This was corrected when the allergies were posted.

An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. There are no infant feeding schedules posted in the infant room.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One inhaler was present at the center that was past the expiration date. Ms. Donaldson removed this item during the visit.

Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. There were several albuterol inhalers on a high shelf in the back classroom. None of the inhalers were accompanied by a medical action plan.

I have four kids that attend this child care facility and they have really grown individually. They really take time with your kids and help to prepare them for school. My kids have learned a lot from this daycare.
