A safe indoor and outdoor environment was not provided for the children. One metal air vent covering in the child care space was observed to be rusting. In the bathroom used by children, one area of the wall near the toilet was observed to have peeling and flaking paint. The outdoor learning environment contains a metal awning used for shade. Rust was observed forming on greater than eight of the bolts securing the structure to the ground, and these bolts were observed to be raised and not flush with the base of the structure, presenting a potential entanglement hazard. | |||

The file for one child enrolled contained a medical report received greater than 30 days after the child had been enrolled. The child was enrolled on 9/20/2022, and the medical report was signed by a physician on 5/19/2023. The file for the operator did not contain the training certificates for Health and Safety training topics #s 8 (Prevention of shaken baby syndrome, abusive head trauma, and child maltreatment) and 10 (Recognizing and reporting child abuse, child neglect, and child maltreatment). The operators Criminal Background Check qualification letter was not observed on file. A permission to administer medication form was not observed to be on file for one child enrolled with an emergency medication.
