A safe indoor and outdoor environment was not provided for the children. In Space 1, the handle on the diaper table was broken, leaving the diaper table unable to be locked, and plastic diaper/wipe wrappers were accessible to children under two years of age, creating a potential choking hazard. Also in Space 1, the cord to a CD player was observed hanging loosely and accessible to children one year of age. In Space 3, a pointed piece of metal was observed to be sticking out from the wall near the Science Center and sharp to the touch. The facility has one playground that wraps around both sides and the back of the building. On the side of the building opposite the side with the shaded area, the fence post that is secured right next to the building leans out slightly, leaving a four-and-a-half inch gap and potential entrapment hazard.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 1, a canister of disinfectant wipes labeled Keep Out of Reach of Children with additional warnings was observed on a shelf less than five feet from the ground. In Space 2, a keychain with protective spray (either Mace or pepper spray) was observed in a drawer less than five feet from the ground in unlocked storage.

The janitor closet containing cleaning supplies was unlocked and accessible to children.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child.
