A safe indoor and outdoor environment was not provided for the children. In Space 1B, the diaper table was unlocked, and plastic diaper/wipe wrappers were accessible to children under two years of age, creating a potential choking hazard. On the playground currently used by children, the top of one section of the plastic fencing was observed to be cracked and sharp to the touch. This section of fencing is not accessible to children of preschool age, but can be reached by children of School Age. This section of fencing was covered during the visit with tape. Plastic weedblock was observed emerging from the ground in greater than three areas of the playground, presenting a potential tripping and/or choking hazard. Along the ramp leading from the building to the playground, one nail and one screw were observed protruding from the base of the ramp; each of these was hammered down to make them inaccessible to children during the visit.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 1A, a small bottle of hand sanitizer labeled Keep Out of Reach of Children was observed on a keychain stored in an unlocked drawer less than five feet from the ground. This was placed greater than five feet from the ground during the visit. In the bathroom located in Space 1B, one can of aerosol room freshener was observed less than five feet from the ground and in unlocked storage. In Space 2 and in the First Aid kit on the vehicle that transports children, the First Aid Kits were observed to contain alcohol prep pads labeled Keep Out of Reach of Children with additional warnings. One drawer of a teacher’s desk was observed to be unlocked and contained an aerosol disinfectant spray and hydrocortisone cream.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Present children had not been signed in each room.

The center did not have developmentally appropriate equipment and materials accessible daily. The space 2, the book area was missing two items, the block area was missing two items, and the home living area was missing two items. These were corrected during the visit. In space three, the book area was missing one item. This was corrected during the visit. In space 1A, the art area was missing one item, and the book area was missing one item. The toys were not together in some classrooms and were not accessible in some classrooms.

The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There were not enough items for three children to play in the art, book, home living or block areas in space 2. This was corrected during the visit. Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. There is one broken knob on the play stove in space 1A.

A safe indoor and outdoor environment was not provided for the children. Children had access to several plastic gallon storage bags during the visit because this is where some toys were housed. These were removed during the visit. Children have access to plastic on the playground where is being used as weed block and is uncovered and ripping in certain areas.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet was uncovered during the visit and this was corrected.

Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. There were nine children who were listed on the transportation roster who did not have emergency information in the vehicle. There was not permission to transport three children, and only one child's form included a photo.

Center did not maintain a record of daily attendance. Attendance had not been taken in the classrooms until after lunch and the daily attendance records for each classroom are not separated or correct. There were more children on the attendance sheet than the teachers reported were enrolled. MS. Herron took attendance during the visit, however the attendance sheet must be corrected and separated.

The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. The safe sleep policy posted in the infant room was blank and did not have the required information.

A safe indoor and outdoor environment was not provided for the children. There are piles of leaves, exposed plastic (weed covering material) and a loose picket on the railing in the playground area.
