Noncompliance Area: During renewal inspection on Friday, 02/13/26, cert rep observed facility failed to have a written plan identifying the means of transporting a child to emergency care and staffing provisions, posted in the facility.

Correction Required: A written plan shall be conspicuously posted which identifies the means of transporting a child to emergency care and the facility staffing provisions in the event of an emergency. The plan must accompany a staff person who leaves the facility on an excursion with children.

Noncompliance Area: In review of health and safety documentation, facility's policy shaken baby syndrome, abusive head trauma and child maltreatment failed to address the prevention and identification of child maltreatment.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: In review of the facility's emergency plan, plan failed to address where staff and children will shelter in place and lock-down procedures.

Correction Required: The facility shall have an emergency plan that provides for accommodations for shelter of children during an emergency including lock-down, shelter in place at the facility and shelter at locations away from the facility premises.

Noncompliance Area: During Initial Inspection on 09/08/2025, Certification Staff observed that the facility did not have a written policy on file that included recognition of potential signs and symptoms of shaken baby syndrome and abusive head trauma, Strategies for coping with a crying, fussing, or distraught child, or addressing the prevention and identification of child maltreatment.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: During Initial Inspection on 09/08/2025, Certification Staff observed that the facility's emergency plan was not sent to the local municipality and to the county emergency management agency.

Correction Required: The operator shall send a copy of the emergency plan and subsequent plan updates to the local municipality and to the county emergency management agency.
