Noncompliance Area: During the renewal inspection, certification representative, reviewed 5 children's files and the file for child #2 did not include an initial health assessment or immunization record.

Noncompliance Area: During the renewal inspection, certification representative reviewed 5 children's files and the file for child #2(ADMISSION DATE 4/14/26) did not include a record of immunizations and the child is still enrolled at the facility.

Correction Required: The facility may not accept or retain an infant 2 months of age or older, a toddler or a preschool child at the facility for more than 60 days following the first day of attendance at the facility unless the parent provides written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of the dates (month, day and year) the child was administered immunizations in accordance with the recommendations of the ACIP. The facility shall implement dismissal policies in accordance with the Department of Health regulation in 28 Pa. Code § 27.77 (relating to immunization requirements for children in child care group settings).

Noncompliance Area: During the renewal inspection, certification representative reviewed the facility files and there was no record of current liability insurance.

Correction Required: The legal entity shall have comprehensive general liability insurance to cover the persons who are on the premises. A current copy of the insurance policy shall be on file at the facility.

Noncompliance Area: During the renewal inspection, certification representative reviewed the facility paperwork and there was no fire system inspection documentation that showed the fire alarm system is in operating condition.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children. Fire detection devices or systems must be in compliance with standards established under section 1016(c) of the act (62 P.S. § 1016(c)).

Noncompliance Area: During the renewal inspection, certification representative reviewed 3 staff files and the files of staff #1 did not have the minimum 12 hrs. of annual childcare training that is required.

Correction Required: A staff person shall obtain an annual minimum of 12 clock hours of child care training.

The facility shall require the parent to provide updated written verification from a physician, physician's assistant, CRNP, the Department of Health or a local health department of ongoing vaccines administered to an infant, toddler or preschool child in accordance with the schedule recommended by the ACIP.

Noncompliance Area: During the renewal inspection, certification representative reviewed 5 children's files and the file for child #4 did not include signed parental consent for the administration of minor first aid by facility staff.

Correction Required: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

Description: Age appropriate

Noncompliance Area: During the initial inspection, certification representative conducted a facility walkthrough and there was not adequate toys/activities for children in the facility or play space.

Correction Required: Play equipment and materials appropriate to the developmental needs, individual interests and ages of the children shall be provided in sufficient amount and variety to preclude long waits for use.

Noncompliance Area: During the initial inspection, certification representative revied 2 staff files and the facility's emergency plan. The file for staff #2 did not include emergency plan training and the emergency plan did not address specific staff duties, evacuation or how the children would be transported to the relocation address.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children.

Description: Age and Training/Health and Safety Training - Pediatric First Aid and CPR

Noncompliance Area: During the initial inspection, certification representative reviewed 2 staff file and the file for staff #2 did not contain a PQAS certified pediatric cpr and first aid training.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification. Staff persons shall complete professional development in pediatric first aid and pediatric cardiopulmonary resuscitation within 90 days of hire.
