All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Mulch beneath the rock wall and the climbing structure measured between 4-1/2 ad 5 inches in some areas instead of the required 6 inches.

All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Staff took a BLS CPR course through the American Heart Association on 4/11/23 which did not include the required documentation verifying the course covers the required First Aid topics.

A safe indoor and outdoor environment was not provided for the children. Two rusted bolts were observed secured in a cinderblock wall. A small, cracked crevice, sharp to the touch, was observed to the left of the stairs near the base of the building. Approximately seven small holes were observed in the cinderblock walls of an outdoor play area at the children’s level, presenting a safety hazard, as a child’s finger to become lodged in a hole.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Wrinkle Releaser labeled Keep Out of Reach of Children with additional warnings was observed in Space 4 in unlocked storage. A small spray bottle of anti-fog mask cleanser labeled Keep Out of Reach of Children was observed in unlocked storage in a teacher's desk drawer in Space 1. A bag of fertilizer mixed with potting soil labeled Keep Out of Reach of Children with additional warnings was observed on the playground behind a small latched gate in unlocked storage.
