For children under three years of age, materials were not offered in sufficient quantity to allow all children to use them at some time during the day and to allow for a range of choices. In Space 2, a sufficient quantity of blocks and musical toys was not available for the children’s use.

A safe indoor and outdoor environment was not provided for the children. A rusted nail was observed protruding from a windowsill to the left of the fenced-in air conditioning unit (when facing the building). A small piece of metal, sharp to the touch, was observed screwed into the base of the brick net to a gutter on the left side of the building (when facing the building). The space between each of the slats of the white fencing bordering the toddler playground measures 3.75 inches in length, creating a potential entrapment hazard. Toddlers could also reach through these slats and have access to small pebble surfacing located along the border of this fencing on the playground for preschool children that could present a choking hazard. Two tricycles were observed to have handles that were covered with mildew.

All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 3, a First Aid kit was observed to contain alcohol wipes, antiseptic wipes, eye rinse solution, and a cooling pack, all labeled Keep Out of Reach of Children with additional warnings. On the van used for transportation of children who are School Age, the first aid kit was observed to have a cooling pack labeled Keep Out of Reach of Children with additional warnings.

All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. I monitored the files for one new staff member and for one existing staff member. The new staff member was employed on 8/02/2022 and has not yet completed CPR/FA certification training.
