A safe indoor and outdoor environment was not provided for the children. On the playground, greater than two metal fence posts were observed to be rusting at the top and base of the posts. One outdoor art easel (on the right, facing the fencing) was observed to have two screws that are rusting. One plastic lid to the sand and water bins was observed to be broken, cracked, and sharp to the touch.

A safe indoor and outdoor environment was not provided for the children. At all corners of the playground where the fencing meets, and at all gate junctions where the gate doors either connect with each other, or with the fencing itself, greater than twenty (20) bolts which secure the fencing were not flush with the nuts and were extending inward from the gate, up to an inch beyond the nuts, presenting protrusion hazards. The screws securing the base of two outdoor easels were observed to be rusting.
