There were not three different activities available to children in the book/language center in spaces 104 or 105. Writing paper and enough pencils for more than three children were added to the center during the visit.

In room 104A, I observed a file cabinet with the key stored in the lock contained an aerosol can and a canister of disinfectant wipes. The cabinet was locked and the key removed to a hanger during the visit.

In room 103A, I observed one diaper cream stored was not labeled with the child's name. The name was written on the diaper cream during the visit.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. I did not observe signed acknowledgment of the SBSAHT policy in one staff file. The staff received the policy and signed acknowledgment during the visit.

A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file for one therapist who was on-site. The therapist was providing services for a child at a table located in the hallway. The therapist provided a valid qualifying letter while on-site and a copy was given to the director. I confirmed the therapist was qualified in regulatory.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. I observed three uncovered outlets on the power strip located in room 107A. The outlets were covered during the visit.

I observed a canister of disinfectant wipes and an aerosol can that were stored unlocked in room 104A; I observed wet mop clothes containing more than one warning stored on top of the refrigerator in room 103A. All items were placed in locked storage during the visit.
