| A safe indoor and outdoor environment was not provided for the children. Indoors, no safety issues were observed. Outdoors, I observed 10 plastic sod, ground cover stakes in the grassed area of the playground. Two of the stakes were pulled out of the ground, exposing two sharp prongs on each stake. During the visit, all 10 stakes were removed from the play area. | |||

| Documentation of staff's on-going training was not on file and/or was not current. I did not observe that staff training was documented on a log in the staff file. Training certificates were kept in the staff files. A training log was added to the file during the visit. | |||
