| A safe indoor and outdoor environment was not provided for the children. Indoors, no safety issues were observed. Outdoors, I observed 10 plastic sod, ground cover stakes in the grassed area of the playground. Two of the stakes were pulled out of the ground, exposing two sharp prongs on each stake. During the visit, all 10 stakes were removed from the play area. | |||
