I have used this provider for more than 6 months

This is the worst day care I've ever seen the director curses in front of the children and and she always have too many kids there with only one she only supposed to have 9 with a teacher and there is 12 to 14 kids dare with one teacher anytime I need to ask questions she's never available I went to pick up my children and she had took the older kids off with her in her car and I don't think that's very safe and my child says they sit upstairs and watch TV

I had the opportunity of taking my grandson to this daycare. It is an excellent daycare I love it!!! The staff is friendly and professional and compassionate. The daycare is bright and colorful and very clean. They also give the kids truly home cooked nutritional meals in which spoke volumes to me. I have had my granson since he was 2weeks old and I rate this daycare the BEST yet. Thank You Happy Days Child Care

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. Peaches were served for breakfast this morning but not added to the posted menu.

All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. One new staff person did not have a Health Questionnaire on file.

Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Ms. Cooper was unable to locate the Emergency Preparedness Plan complete on the template provided by the Division. The Plan that she located was not dated.

The person identified as the person or alternate person responsible for carrying out the emergency medical care plan was not on the premises at all times and/or did not accompany children for off premise activities. Ms. Cooper and Mr. Crawford are the only persons listed on the Emergency Medical Care Plan and neither were present at the child care when I arrived.

The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. In the book area, there were books and a puppet stage available to children. The puppets were in a different part of the room. When I told Ms. Cooper that she must have three different activities in each center, she added an alphabet puzzle to the area.

Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu for last week had fallen off the wall and was not posted. This week's menu was posted during the visit.

Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. Two tricycles on the playground were missing the handles.

There was not a sufficient supply of clean linen on hand. The children laid down for nap on mats with no linens. When I reminded Ms. Cooper that she must have a sheet on each mat, she stated that she received incorrect information about this rule and she put a sheet on each mat immediately.

A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. The First Aid Sheet was posted only in the back classroom. Ms. Cooper posted it on the main board during the visit.
