Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A refrigerator thermometer registered at 50 degrees in the room for infants where bottles were stored. The thermometer in another refrigerator in the room for infants where bottles were also stored was removed during the visit and could not be located.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A plastic bag was found in a child's cubby within children's reach in the classroom for older two year old children.
